In the interest of protecting the health of our employees, customers and other visitors to SRT Group facilities, SRT Group is implementing certain precautionary measures to help mitigate against the spread of COVID-19, including requiring all visitors to validate certain data prior to visiting our sites.
Part I:
We require that you respond to this request as instructed below at least 48 hours in advance of your planned visit to a SRT Group facility. No visitors will be authorized to enter SRT Group facilities without answering the questions below.
Part 2:
If local requirements do not require otherwise, if you are a fully vaccinated individual, as defined by the CDC, and if you desire not to wear a face mask or physically distance during your visit at SRT Group, please complete Part II.
